Complex care services refer to a range of healthcare and support services that are designed to meet the needs of individuals with complex medical, social, and/or behavioral needs. These individuals typically require ongoing, specialized care and support that cannot be adequately met through traditional healthcare services.

Examples of individuals who may require complex care services include those with chronic conditions such as diabetes or heart disease, individuals with disabilities or developmental disorders, and individuals with complex mental health needs. 

Our complex care package would involve, but not limited to:

  • Coming up with a customized live-in care plan to meet all your specific needs.
  • Coordinating with necessary specialists when required to make sure that you are receiving the needed support at all times.
  • Establishing a point of contact with a necessary family member and specialist for regular updates on your progress.
  • Providing a care expert who suits your specific needs and personality.

Health Issues that Require Complex Care

Chronic illnesses such as diabetes, heart disease, kidney disease, cancer, etc.
 
Neurological disorders such as Parkinson’s disease, multiple sclerosis, ALS (amyotrophic lateral sclerosis), etc.
 
Mental health conditions such as bipolar disorder, schizophrenia, major depression, etc.
 
Traumatic injuries such as brain injuries, spinal cord injuries, severe burns also require complex care.
 
Chronic pain such as fibromyalgia, chronic back pain, arthritis can be difficult to manage and require complex care.
 
End-of-life care for patients with terminal illnesses requires complex care that involves managing pain, providing emotional support, and helping patients and their families.

2. Care Team Assessment

We will then arrange for one of our experts to visit you and carry out a comprehensive assessment of your care needs. After this visit, a fully costed care package will be offered and a plan created in collaboration with you and your family to ensure that you are happy with all aspects.

3. Selecting Your Care Giver

We will identify suitable healthcare professionals to support you in-line with your care plan and arrange for you to meet them to ensure that all of your needs are met. If any additional training is required to support your care requirements, this will also be coordinated.

4. Care Begins
